Consumer Complaint: In XXXX of 2014 I was seeing a XXXX in XXXX XXXX, DE for XXXX XXXX XXXX. After several weeks of treatment the XXXX sent me to get an XXXX done at XXXX XXXX XXXX XXXX XXXX in XXXX, DE. The results showed that I had XXXX XXXX at three areas on my XXXX. I continued treatment at the XXXX. I am now on a wellness plan with a different XXXX 4 years later and attend monthly appointments unless there is a flare-up in which case I attend more weekly. I was billed {$2000.00} for this XXXX but was not aware that is was on my credit report. I had health insurance at the time with XXXX and since my XXXX sent me with a referral I do not believe I should have to pay the full amount for the XXXX. It is detrimental to my credit report, but I was unaware. I believe since I moved around a lot the bills were not being sent to my correct address and so I did not fight it back in 2014.

Consumer Complaint: I opened two wellness accounts with the XXXX XXXX Hospital for my two dogs. First was opened on XX/XX/XXXX, second on XX/XX/XXXX. Every visit was paid the same day before we left the hospital and on top of those payments the XXXX was collecting monthly charges of {$64.00} every month on first for the first dog and {$42.00} for the second dog on XX/XX/XXXX every month by withdrawing it from my checking account and those funds were later used to pay for some charges for the dog 's treatments and wellness visits. Attached are all the bills and payments showing that everything was paid on time. Unfortunately my dog had to undergo surgery to remove the XXXX in XX/XX/XXXX. XXXX was paid in full. The outcome was very bad, XXXX XXXX XXXX XXXX was XXXX and XXXX, XXXX were nXXXX XXXX XXXX together and XXXX XXXX. The vet XXXX XXXX XXXX second time but it did not help and XXXX XXXX again. I took our dog to the other clinic and was told that it was XXXX XXXX, dog was in very bad condition with XXXX XXXX XXXX and whoever did the XXXX did not do it the right way. We were told that the only thing we could do now was to XXXX the dog. So we did. I called XXXX XXXX to cancel the membership on XX/XX/XXXX and asked them to stop withdrawal from my checking account but was told that it is impossible since I had to pay club membership for entire year from starting it, dead dog or not, coming for appointment or not. They kept withdrawing funds for the wellness club for BOTH dogs for the next several months until I closed my account due to identity stolen and money stolen from the account. At this point XXXX sold both dogs ' account to IC System and on XX/XX/XXXX they started collection activity that I have on my credit till now. They want {$300.00} for the dog that actually was killed by their veterinarian and {$160.00} for the other dog that was already under other veterinarian care. Like I mention all bills were paid at the time of their visit, and I even received refund for the overpayment for the dog that was later euthanized. My surviving dog had different veterinarian and I was paying her bills at the new place. I was not going to take my other dog to the place that killed my other one.

Consumer Complaint: Creditor is reporting accounts that are invalid and unverified. XXXX and XXXX have investigated this account and they concluded the account was not matching my records of being assigned to me. I would like proof these accounts were disputed per FCRA/ FDCPA. This is causing me anguish and its slandering my good name. Please block this account from my report so I can breathe again.

Consumer Complaint: Ive sent out certified letters to XXXX XXXX XXXX for a collection account thats reporting {$840.00} of false debt!! I have never received any info on this debt! They will not respond to my letters! maybe because their physical address theyre reporting is a non deliverable address and so I have sent letters to their P.O. Box and someone has picked them up but Im afraid that the P.O. Box is someone elses and now they have my SS number and full name as well address!! The post office screen said INVALID address for their physical address!! I wasnt aware of this on my credit until This XX/XX/XXXX! all this time this false collection company is reporting inaccurate information!! Ive asked for documentation with my signature but have yet to receive anything!!

They have not. I have asked the collection agency to do the same. No response in writing has been provided. I can not imaging owing this debt since I currently have active service with the provider. It was mentioned at the time I applied.
