Consumer Complaint: Recently while conducting a monitor of my credit report I discovered that I C SYSTEM COLLECTIONS has furnished some items that have unverified information. I believe under the Fair Debt Collections Practices Act, I have the right to request that you allow me to validate the alleged debt. I am aware that there is a debt from I C SYSTEM COLLECTIONS but I am unaware of the amount due and your bill does not include a breakdown of any fees. Additionally, I am allowed under the HIPAA law [ HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 ], to protect my privacy and medical records from third parties. I do not recall giving permission to XXXX XXXX XXXX to release my medical information to a third party. I am aware that HIPAA does allow for limited information about me but anything more than XXXX XXXX XXXX is to only be revealed with the patients authorization, therefore my request is two fold and as follows, Validation of Debt, and HIPAA authorization.

Consumer Complaint: 1 ) Dentist performed and completed dental work without authorization of insurance. Dentist offered me bridge and ssid it was covered after it was done insurance denied it due to previous tooth missing. Dental then charging me for full cost. Insuance advised me not to pay since dentist did not receive authorization prior to work. Had I known it was not covered by insurance I woul have had a single crown which would have been covered. 2 ) Went to dentist to re glue a crown and covering dentist filed crown and exposed metal on different areas then glued back on. I then went to my regular dentist and told her she lied to me and ssid that was just normal wear and tear! No it is not it was obvious of filing. Besides whennI went in crown was intact and notice at home it was exposing areas of metal! Terrible services and no honesty! Very unsatisfied! Poor communication
