Consumer Complaint: I have been receiving several calls and mail from " I.C. System, Inc. '' with regard to an 'equipment balance due ', naming the creditor as XXXX . However, I never had such a charge from XXXX. I moved from XXXX to XXXX but returned my equipment on time and in person and am still a customer of XXXX ( with perfect payment history at that ). Consumer Complaint: I had to paid XXXX XXXX XXXX and open a new account I had a {$57.00} balance on my account with XXXX XXXX XXXX ; I had XXXX XXXX XXXX for about two years ago. I had stopped the service for a while and then started back up, but they said I needed to pay the {$57.00}. I paid it directly to XXXX XXXX XXXX, but I was still getting notices from I.C. System Inc. saying that I owed this amount. This information was removed from XXXX, but not from XXXX.

Consumer Complaint: Recently while conducting a monitor of my credit report I discovered that items were furnished that have unverified information. I believe under the Fair Debt Collections Practices Act, I have the right to request that you allow me to validate the alleged debt. I am aware there is a debt from but I am unaware of the amount due and your bill does not include a breakdown of any fees. Additionally, I am allowed under the XXXX law [ HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT OF 1996 XXXX, to protect my privacy and medical records from any third parties. I do not recall giving permission to release my medical information to a third-party. I am aware that HIPPAA does allow for limited information about me but anything more is to only be revealed with the patients authorization, therefore my request is two-fold and as follows, Validation of Debt, and XXXX authorization. 1. Please provide breakdown of fees including any collection costs and medical charges 2. Provide a copy of my signature with the provider of service to release my medical information to you. 3. Cease any credit bureau reporting until debt has been validated by me. 4. Please send this information to my address listed above and accept this formal complaint as my formal debt validation request, which I am allowed under the FDCPA. Please note that falsification of the information you received from any medical provider can be perceived as a XXXX compliance violation as well as a violation of the FDCPA because you will be deceiving me after my written request of debt validation. I request full documentation of what you received from the provider of service in connection with this alleged debt. Additionally, any reporting of this debt to the credit bureaus prior to allowing me to validate it may be a violation of the Fair Credit Reporting Act, which can allow me to seek damages from a collection agency. Consumer Complaint: I.C system has been trying to collect a debt I do n't owe.The original owner of the debt was XXXX.A power provider that advertises its services without the need of a contract or even credit check.My obligation to XXXX was fulfilled and no balance was due upon the termination of service. However, XXXX claims that a contract was entered and an early termination fee is due.Such charge was disputed and removed from my credit report back in XXXX of 2015. However, I.C system is once again trying to collect on a account they know to well is satisfied.Furthermore, they 're once again reporting negative information to the credit bureau.
