Consumer Complaint: IC System is reporting incorrect negative information on my credit file.I recently received a copy of my credit file and noticed that IC SYSTEM is reporting a collection on my credit from XXXX. According to the Fair Credit Reporting Act ( hereafter referred to as FCRA ) Section 609 ( a ) ( 1 ) ( A ), You are required by federal law to verify through the physical verification of the original signed consumer contract all accounts you post on a credit report. Otherwise, anyone paying for your reporting service could fax, mail or email in a fraudulent account. Therefore, I am demanding to see verifiable proof ( an original Consumer Contract with my Signature on it ) you have on file of the accounts listed below. Your failure to positively verify these accounts has hurt my ability to obtain credit. Under the FCRA unverified accounts must be removed and if you are unable to provide me a copy of verifiable proof, you must remove the accounts listed I am absolutely certain that the item is incorrect and should be removed, so I am hereby exercising my rights under FCRA 611 ( a ) ( 7 ) to request a complete description of all methods used to investigate my aforementioned dispute. I am asking for this verification because my credit score is important to me, and I believe it is being unjustly degraded as a result of this unfortunate error. I would therefore request that you please do not send me a template letter in response to this request. I am in the process of planning a legal case, so I need specific answers to the specific questions asked of you in this letter

Consumer Complaint: I C Systems, Inc., located at XXXX XXXX XXXX XXXX , XXXX XXXX, MN XXXX, is in violation of the Fair Credit Reporting Act. I C Systems, Inc. placed a negative collection account on my credit reports, that I have no knowledge of, without notifying me before placing the collection account. I was not allowed to validate this alleged debt prior to the company reporting it to the credit bureaus which is substantially damaging my credit. After doing some research, I found the original creditor, XXXX XXXX XXXX, a company that I also have no knowledge of, is associated with XXXX Hospital ER billing dept. I had only been seen at XXXX Hospital once, on XX/XX/XXXX and that bill was paid in full in XXXX through XXXX and myself. The amount I C Systems, Inc is reporting is the discount the insurance company negotiated with the provider that was applied to the bill. IT WAS NOT A CHARGE. I only owed XXXX Hospital a payment of {$59.00} after insurance paid out which I paid in XXXX. My score has dropped over 60 points due this is company 's failure to perform their due diligence instead of just purchasing random 'alleged delinquent accounts '. Again, {$580.00} is the amount that was negotiated by the insurance company. This company needs to remove this erroneous account that was added to my reports in XX/XX/XXXX since they failed to actually verify and confirm that a balance of {$580.00} was owed.

Issue: Written notification about debt
Subissue: Didn't receive notice of right to dispute
Consumer Complaint: I recently went to the police department to complain and file a report. They asked if before I went further with the report, that I can bring in solid evidence that my identity had been compromised. I then reached out to the bureau 's as well as the original credit numerous times, no documentation provided. Please note, the few times I've reached out it's been in seek of not just an itemized invoice. I then recently sent them my FTC report, it's now the 52nd day and no response. I, unfortunately, did not record the login information and had to update it today. I have no relationship with this collection agency or creditor and this is not my debt. I received no right to dispute, anything! It just appeared on my credit report. I've lived at the same address for 6 years, nothing has come to my home. Full violation of 609 and 823, as well as the FCRA that requires they respond to my FTC affidavit. I say all of this with humility.

Consumer Complaint: I have no knowledge of this account, and am unsure of the debt. I have not received a notification to initiate a dispute for this information, 809 ( a ) of the Fair Debt Collection Practices Act. Under 809 ( a ) ( 3 ) of the act, I know that the 30 days starts upon receipt of notification to dispute. However, I have not even received the information, regardless of when it was mailed out, and it is affecting my credit report. I want this information to dispute the debt, and receive validation of this supposed debt that I have no knowledge of.

Issue: Problem with a credit reporting company's investigation into an existing problem
Subissue: Their investigation did not fix an error on your report
Consumer Complaint: This company claims that I have a balance of {$310.00} with them, I don't know who they are or what it is that this company does. I searched online for some information on them and it says that they collect debts. I sent a letter to XXXX asking them to validate this debt because I do not recognize where this debt came from and all that this company sent me is a copy of my credit report saying that the account was validated without providing me with an original contract bearing my signature. I asked for documents that would show an original contract with my signature or something that binds me to this debt but up to this day I have not received anything like that from them. This company placed this debt on my credit report as in collections and it is affecting my credit worthiness really bad. I know that by law they have to provide me with a contract that has my signature on it. On my credit report it shows ( XXXX XXXX ) as the original creditor but I never had an account or a contract with them. I want to see the contract with my signature on it that says that I agreed to pay this company.

Consumer Complaint: I have contacted I C Systems INC about a debt in the amount of {$630.00} for XXXX pet hospital. The account is not mine and is how it is spealt in XXXX not english form. The account is for a dog named XXXX which I do own but anyone can make up a name for a dog & my neighbours have pets as well in the same apartment complex. I do not know who may have done this but I know without a doubt it was not done with my conscious, present authority at the location the contract signing took place. This is a case of identity theft and I have never opened an account/ entered into a contract nor have I ever signed a contract with my signature with XXXX pet hospital . I assume my brother entered into into the contract and he has paid for the account using his credit/debit card but upon speaking with him he does not remember. I have contacted the representatives and was constantly told " I don't know '' if a police report being submitted will result in removal of the fraudulent debt. XXXX has contacted me and I have told them as well that I have never signed a contract with them. I have submitted disputes to the Credit reporting agencies and a dispute to IC Systems INC. The signatures on the contract will not match my signature on my driver 's licence nor will security camera footage from the day the contract was signed will show I was not there to enter into the agreement. I request this fraudulent debt be removed as it does not belong on my account.
